The Title IV Solutions name and voice remain primary throughout this website. When a streamlined document workflow is better handled by FormHaven, we identify that referral before you choose to continue.

How the current website handles an inquiry

When you submit the contact form, Title IV Solutions stores the name, email address, phone number, selected concern, and message you provide in its private review queue. The submission creates a lead record and a staff follow-up task so your request is not dependent on an email application.

We also process limited technical information, such as a one-way hash of the submitting IP address, browser information, and submission time, to operate and protect the form. Do not put an SSN, FSA ID, password, or financial-account number in the public form. Protected information is requested only through an appropriate secure workflow when it is needed.

Live website assistance

While this website is open, Title IV Solutions uses a limited, first-party presence record to show staff which public page is being viewed and whether a visitor may need assistance. The record can include a random browser-session identifier, page path and title, referring page, browser information, and recent activity time. It does not record keystrokes, passwords, Social Security numbers, financial-account numbers, or the contents of other websites.

If you choose to open the website chat, the name, email address, and messages you submit are saved in the Title IV Solutions CRM so staff can respond and continue the conversation. Sending the first message may create a lead record. Do not send an SSN, FSA ID, password, or financial-account number through public chat.

Optional text-message consent

You may separately choose to receive text messages about your request. Consent is optional and is not a condition of purchase. Message frequency varies; message and data rates may apply. Reply STOP to opt out or HELP for help.

• The tools remain inside a Title IV Solutions experience, with Lentara identified only as the supporting technology.
• Your entries are calculated in this browser and are not automatically sent to Title IV Solutions, Lentara, or FormHaven.
• Nothing is shared with Title IV Solutions or FormHaven unless you deliberately choose a contact or referral action and send information.
• When a situation fits FormHaven, Title IV Solutions identifies the referral before you leave this site. Following a link does not automatically send your tool entries or create a case.
• On plan setup, Title IV Solutions sends only the name, email, phone, and selected service you approve after you accept both handoff consents and choose secure checkout. FormHaven then opens the processing case and provides the payment page.
• Calculator values will not be added to a contact request unless you expressly choose to include them.
